In recent months we have received a number of unsolicited contributions concerning this subject. The publication of the conclusions of a symposium in the February 2000 issue of the American volume of the Journal of Bone and Joint Surgery together with the appearance of the results of the PEP trial in the Lancet suggested that a selection of these papers might be grouped together and the subject discussed in an Editorial.
