Prospective Evaluation of the Therapeutic Interchange of Felodipine ER for Amlodipine in Patients With Hypertension

The objectives of this study were to evaluate the hemodynamic effects, electrocardiographic effects, and cost-avoidance associated with a therapeutic interchange program from amlodipine to felodipine ER. Patients (n = 44) stabilized on amlodipine therapy for hypertension in an ambulatory clinic were included. Patients were switched on a direct mg for mg basis from amlodipine to felodipine ER. Blood pressure variables were recorded during amlodipine therapy and on two follow-up visits after switching to felodipine ER. A 12-lead ECG to determine heart rate and other variables was performed during amlodipine therapy and on the first follow-up visit (n = 30). No changes in blood pressure control, heart rate, or ECG variables occurred after switching from amlodipine to felodipine ER. The estimated annual cost-avoidance for the health system is $15,440. Switching patients from amlodipine to felodipine ER produces similar effects with substantial cost-avoidance.
