Congenital obstruction of only 1 portion of the renal collecting system is almost always associated with duplication anomalies. When the duplication is complete the obstruction usually will be at the ureterovesical junction and when it is incomplete obstruction usually will be at the ureteropelvic junction. Herein we present a case of severe obstruction of the lower portion of the kidney that was drained by a bifid intrarenal pelvis and a single ureter.
