Abstract
Health professionals are responsible for ensuring children and young people access and engage in a purposeful and planned transition from children's to adult services. This article explores policies and guidance on transitional care for children and young people in relation to current and future nursing education at undergraduate and postgraduate level. It also explores how practices could be modified to give children and young people a better transitional experience, and looks at the need for new research and guidance.
