A 43-year-old white woman was referred for evaluation of a chest radiograph showing reticulonodular shadows in the middle lung zones (fig. 1). She complained of a nonproductive cough and mild dyspnea on exertion. She also noted increasing arthralgias, and occasional drenching sweats for 1 year. She denied weight loss. She had undergone a transabdominal hysterectomy and bilateral salpingo-oophorectomy for endometriosis 6 years prior to presentation. She had smoked 1 pack of cigarettes per day for 25 years. She was a registered nurse employed part-time by an oral surgeon. She related that her mother died years earlier from bronchogenic carcinoma.
